For business/acting headshots it's generally a flat rate, which includes very basic simple retouching.
For environmental portraiture or commercial it's generally a day rate plus the cost of retouching on the back end. Prices, however, are variable based upon scope, like the hours necessary to achieve the look, from preproduction, the shoot itself, to the backend.
For Instagram/Facebook short films and advertisements, it's much the same. Generally it's a flat rate, but it is dependent on scope and what is necessary to achieve the desired results.
